Evaluations are completed at the end of each day and are mandatory to receive your certificate.  There will be one evaluation for Thursday,  Friday, and Saturday.

If you attend a break-out session for Domestic Violence, Medical Errors, HIV/AIDS, Telehealth, or Ethics, you must complete a separate sign-in sheet and evaluation for feedback on that session.
