Description: Integrated Care allows providers from multiple disciplines to provide collaborative care to individuals and families leading to early symptom detection and treatment. People with common physical health conditions have higher rates of mental health concerns and those with serious mental illnesses and substance use disorders have higher rates of mortality. The bidirectional implication of mental and physical illness on the individual is well documented by the research literature. Consequently, treating the whole person and addressing both physical and behavioral health concerns leads to positive health outcomes and cost-effective quality care.
